We ask that you log into https://azdpsccw.permitium.com/entry either prior to the class or after we provide you the (2) copies of fingerprints and class certificate; this will alleviate the paperwork portion of the class and give you more time to practice shooting your weapon.

Click View Details under Concealed Weapons Permit Application, Click NEW Concealed Weapon Permit Application, or choose another option if not new. Complete the application; it will ask you to upload a picture of your training certificate, which we will provide, as well as your ID. What to mail: YOU MUST INCLUDE A COVERSHEET THAT INCLUDES THE ORDER NUMBER OF YOUR APPLICATION.  YOU WILL RECEIVE A COVER SHEET AS CONFIRMATION AND CAN PRINT THIS FORM ONCE YOU HAVE SUBMITTED THE CCW APPLICATION. You will then send the cover sheet with your hard copy fingerprints and the certificate we provide you to the address provided. **You will be able to check the status of your application with your log-in. CCW class fee is non-refundable but can be used towards another CCW class.
